MedDRA coding and case assessment

MedDRA provides the standardised terminology that makes safety data comparable across products, companies and regions. Coding decisions have real consequences: choosing an overly general term dilutes a potential signal, while choosing an overly specific term can fragment related events across multiple preferred terms.

Seriousness is a regulatory determination, not a clinical impression. An event is serious if it results in death, is life-threatening, requires or prolongs hospitalisation, causes persistent or significant disability or incapacity, results in a congenital anomaly, or constitutes another medically important condition.

Expectedness compares the reported event against the reference safety information — the company core data sheet or approved labelling. Causality assessment then applies a structured method to judge the relationship between product and event, considering temporal association, dechallenge and rechallenge, alternative explanations and biological plausibility.

Regulatory reporting timelines and compliance

Expedited reporting obligations are timeline-driven and compliance is measured precisely. Serious unexpected adverse reactions generally require submission within fifteen calendar days of the day the marketing authorisation holder first becomes aware of the case, with fatal or life-threatening cases in clinical trials subject to a seven-day requirement for initial reporting.

The clock starts at day zero — the date of first receipt of the minimum information by any employee or agent of the company. This is why intake discipline across commercial, medical information and quality complaint channels is treated as a compliance-critical process rather than an administrative one.

Submissions are made in E2B format to regional gateways, and reconciliation between the safety database, partner exchanges and regulatory acknowledgements is a standing operational responsibility.

Aggregate reporting

Aggregate reports move the analysis from the individual case to the cumulative picture. The Periodic Benefit-Risk Evaluation Report, which has largely replaced the older PSUR format, presents a critical evaluation of the benefit-risk balance of an authorised product over a defined interval, integrating clinical trial data, post-marketing exposure and literature.

The Development Safety Update Report performs the equivalent function for products under clinical development, while the Periodic Adverse Drug Experience Report remains a United States requirement for certain applications.

Signal detection and risk management

Detection combines quantitative methods — disproportionality measures such as the proportional reporting ratio and information component applied to spontaneous report databases — with qualitative medical review, which remains essential because rare but serious events may never reach statistical thresholds.

The Risk Management Plan documents the safety specification, the pharmacovigilance plan for further characterisation, and the risk minimisation measures — labelling changes, educational materials, controlled access programmes — together with the means of measuring their effectiveness.

Building a pharmacovigilance career

Drug safety offers one of the clearest structured entry routes in the pharmaceutical industry for life science and pharmacy graduates. Careers typically begin in case processing, progress through medical review and quality review, and then branch into aggregate reporting, signal management, risk management, compliance or system ownership.
